Function nextLaneToProvision

    input: {
        env?: DemandEnv;
        provisioned: number;
        rows?: RowBudget;
        windows: readonly DemandWindow[];

The next lane to provision, or undefined when nothing should be. An explicit REPLICA_COUNT is a floor; REPLICA_AUTOSCALE=0 pins the number instead.
